Changes to input forms must be approved by your Company Admin and will usually be applied by your WellEz Account Manager. If you do not know either of these you can submit your request to Support by emailing firstname.lastname@example.org or calling WellEz Support at 281-584-9200 (option 1).

- What reports you would like to view this change with
